Home care education stream

Home care is a vitally important part of the Canadian health system. Home care plays a key role in areas such as managing chronic disease across the care continuum, from clinic to hospital to home, allowing patients to come home as quickly as possible after hospital stays and supporting seniors in living safely at home longer. Here are highlights of the education available for home care professionals at e-Health 2015.
